Congregational Care Request Form

Congregational Care is how we care for one another and is rooted in Hillhurst's values of hospitality and spiritual nurture.  If you are a congregant of Hillhurst or Living Spirit and in need of short-term support, connection or encouragement, please fill out this form and one of our compassionate volunteers will be in touch with you.  

Please select "other" if you are uncertain of the need. All requests will be reviewed by the care team leader.
